Half.

That's about the risk of dying — over years of follow-up — for adults who did two ordinary things, strength training plus cardio, set against those who did neither, across 500,705 people. One caveat up front, because it matters most here: this compares people who chose to do both with people who didn't — an association, not a controlled trial. Read it as a strong signal, not a promise. But it keeps turning up.

For a month this newsletter has taken apart the supplement shelf — GlyNAC, taurine, magnesium, vitamin D. This week we put the bottle down, because the best-evidenced intervention I've covered in four years was never on it. You can't buy it. You can only build it.

🔬 Top 3 Interventions Under the Microscope

The intervention with the strongest evidence in this whole archive doesn't come in a bottle. Here's the honest grading of the thing itself — and the two supplements that genuinely earn a place beside it.

1. Resistance training — Evidence Grade: A

What it is. Progressive resistance training — lifting a load heavy enough that the muscle has to adapt, then making it a little heavier over time. Muscle isn't just meat for moving you around; it's your largest glucose sink and a reserve you draw on when illness or age starts making withdrawals.

Human evidence. A 2022 meta-analysis by Momma and colleagues in the British Journal of Sports Medicine pooled sixteen prospective cohorts and linked muscle-strengthening activity to roughly 15% lower all-cause mortality and 17% lower cardiovascular death — with the curious twist that most of the benefit arrived by 30 to 60 minutes a week, then flattened. The "half" from the welcome is López-Bueno's 2023 study in JAMA Internal Medicine: half a million people, the lowest risk belonging to those who combined lifting with cardio. Lift alone, stripped of the cardio, had a softer independent signal — which is exactly why the causal question is worth taking seriously, and why it gets this week's Deep Dive.

Cautions. The risk isn't the training, it's skipping the on-ramp. New, deconditioned, or managing heart disease or uncontrolled blood pressure? Start with bodyweight or a supervised session. Ego is the main injury mechanism.

Takeaway. Strength and function gains are RCT-solid; the mortality link is the most consistent observational signal in the field. It's the closest thing to a sure thing I've found in four years of writing this. Grade A — and the one recommendation here that outweighs everything else combined.

2. Protein in older adults — Evidence Grade: B

What it is. The official adult requirement — 0.8 g per kilogram of bodyweight a day — is enough to stop you wasting away, not enough to defend muscle against the decades. Ageing muscle also develops anabolic resistance: it responds less to a given dose, so older bodies need more.

Human evidence. Morton's 2018 meta-analysis (49 trials, 1,863 people) found protein added about 0.30 kg of extra lean mass on top of resistance training, topping out near 1.6 g/kg/day. Take the training away and the effect goes with it: a 2025 umbrella review in Age and Ageing, spanning 33 meta-analyses, found protein alone did little for muscle or strength in healthy older adults. Protein amplifies a stimulus; it doesn't replace one.

Cautions. Very high intakes warrant caution with kidney disease. And much of this literature sits close to the dairy and supplement industry — food-first is the honest reading.

Takeaway. If you're training, aim for 1.2–1.6 g/kg/day. If you're not, the extra scoops mostly feed your imagination. Grade B.

3. Creatine monohydrate — Evidence Grade: B

Creatine tops up phosphocreatine, the fast-access battery your muscles drain in the first seconds of hard effort — a fuller battery, a rep or two more, and over months those reps add up. A decade of meta-analyses agrees on a small, consistent effect: Forbes' 2021 review (16 trials, people over 50) found creatine plus training added about 1.3 kg of lean mass over training alone. A 2025 trial in 63 lifters found no extra lean mass at all — a fair reminder the effect is modest and not universal. Weighed against the larger body of work, I read it as "small but real," not "debunked."

Cautions. The kidney worry is mostly a misunderstanding: creatine nudges up blood creatinine, a marker, without harming the kidney itself (genuine renal disease aside — ask first). Skip the fancy "buffered" forms.

Personal note: I've had 5 g of plain monohydrate in my morning coffee for years. About £4 a month, and it doesn't pretend to be anything it isn't.

Takeaway. Small, real, cheap, safe, best paired with actual lifting. Grade B.

SPOTLIGHT

⭐ Spotlight: Do You Need to Time and Space Your Protein?

This is the one place I'll get into the protein weeds, because the optimisation culture around it is where a lot of money and worry goes. Open any fitness app and you'll be told to hit 30 grams every three hours, never miss the post-workout window, and spread protein evenly or forfeit gains. The science underneath is real — up to a point.

Pros. Older muscle does need a bigger per-meal hit to switch on protein synthesis: Moore's 2015 work put the threshold near 0.4 g/kg per meal, almost double the young figure, with a leucine trigger inside it — around 2.8 g of leucine per meal in older adults, versus the 1.7 g that didn't move the needle in Katsanos's 2006 study. The per-meal threshold is worth knowing.

Cons. What doesn't hold up is the stopwatch. When an 8-week trial fed older adults the same daily protein either evenly or skewed across meals, the distribution pattern changed nothing — not lean mass, not strength, not function. Total daily protein was the only thing that moved. And the sacred post-workout "anabolic window"? Schoenfeld's meta-analysis found its effect evaporated once you adjusted for total daily protein. The window is hours wide, not minutes.

Clear the per-meal threshold at a few meals, hit your daily total, and the timing takes care of itself.

Bottom line: ⚠️ Real, but oversold — the per-meal threshold is worth hitting; the minute-timing and even-spacing rules aren't. Hit your daily total across a few solid meals and stop counting minutes.

DEEP DIVE

📖 Deep Dive: The Vital Sign Nobody Measures

Your grip strength predicts your risk of dying more sharply than your blood pressure does. A five-kilogram drop in what a hand dynamometer reads tracks a measurable rise in mortality — and almost nobody reading this has ever had the number taken.

Which sounds like the perfect argument for lifting, until you hit the catch: does building strength push your risk down, or does strength just reveal who was healthy all along? That question decides whether any of this is worth your time, and the mortality statistics can't answer it on their own. This week I work through what can.

HYPE CHECK

🚨 Hype Check: The 'Muscle Preservation' Powders

The Hype: A whole shelf sells muscle protection in a scoop — HMB, BCAAs, EAAs, "anti-catabolic" blends. The capsules aren't cheap: HMB runs £20–37 a month, BCAA and EAA tubs £20–28.

The Evidence: It splits by who you are.

Trained and well-fed: close to nothing. Sanchez-Martinez's 2018 meta-analysis (six trials, 193 athletes) found effect sizes near zero, and when a 2019 trial pitted HMB against the plain leucine it's made from, they tied. BCAAs supply only three of the nine amino acids muscle needs — about half the building response of a complete protein. The dramatic early HMB results came mostly from studies run by the patent-holders, and haven't held up in independent trials.

Older, frail, or genuinely wasting: a small real signal — roughly 0.28 kg of lean mass in a 2025 review. A mild brake on muscle loss, not an engine for building it.

Why It's Misleading: "Anti-catabolic" sells you one half of a ledger — these compounds nudge breakdown down, but nudge building down too, so the net barely moves.

Our Verdict: For the well-fed lifter, redundant: eat enough protein and lift, and you already have every signal these imitate. If you're older, at-risk, and want to trial HMB, buy the £3.80-a-month powder, not the £37 capsules — same molecule, a tenth of the price.

SUPERFOOD

🥚 Superfood Spotlight: Whole Eggs

One small, cheap thing on the way to the protocol: eat the yolk, not just the white. In a 2017 trial, whole eggs drove roughly 40% more muscle protein synthesis after a workout than the same amount of protein from egg whites — something in the yolk does work the white can't. About 6 g of complete protein an egg, roughly £3 a dozen, and moderate intake is fine for most on the current cholesterol evidence.

BIOHACKING CORNER

🌡 Biohacking Corner: The Whole Issue, in a Week

If you read nothing else, this is the usable version.

1. Lift twice a week, and add load over time. Six to eight movements — legs, a push, a pull, something you carry. Progress beats novelty: add a rep or a kilo before you add an exercise.

2. Eat enough protein. Roughly 1.2–1.6 g/kg across three or four meals, each with a decent slug. Don't stopwatch it.

3. Add 5 g of creatine if you want the one cheap edge — no loading, no timing.

Thirty to sixty minutes of lifting a week is where most of that lower risk clustered in the data — then it flattened. You are not required to live in the gym.

Caveat: new to training, pregnant, or managing a heart condition, uncontrolled blood pressure, or kidney disease? Get cleared and start supervised.

Four issues auditing the supplement shelf, and here's the part that still catches me: the strongest thing on the table was never on the shelf. Lift, eat enough protein, add creatine if you like — that's the muscle protocol, and it's very nearly free.

But muscle is only half of that opening number. The other half is your engine — so Issue 22 takes on VO₂ max, why it may be the single strongest predictor of how long you last, and whether Zone 2 deserves the hours everyone gives it.
